Submit Physical Form

All athletes must have a current sports preparticipation physical exam on file in the Nurse's Office.  Physicals are good for 1 calendar year from the exact date of your last physical.  You can get the blank Physical Form here.

Once you have your physical completed by a physician, you can turn it in to the athletic office or you can scan it and submit it electronically below.
